To determine if your chosen physician or this office is on your plan please contact your insurance member service.  As a reminder, if TS&FM is on your insurance plan we will file the claim as a courtesy to you. If we are not on your insurance plan then the visit will be fee for services, payment expected the day of the visit.  At the conclusion of your visit you will be given a statement that you can then file for out of network benefits
